Synopsis: This conference focuses on improving anticoagulation therapy services with a focus on improving patient safety and new oral anticoagulants.

The conference opens with a presentation on quality and safety indicators for anticoagulation therapy. The conference continues with case studies on learning from incidents and near misses and proactive tools to improve safety in anticoagulation therapy. The afternoon sessions focus on improving practice and safety in the use of new oral anticoagulants including managing new oral anticoagulants in your services, patient information and education, educating frontline staff in trigger tools and recognition of patients on new oral anticoagulants and reducing Medication Errors and ensuring patients receive oral anticoagulation therapy within safe levels. The final session looks at commissioning anticoagulation therapy including the commissioning of new oral anticoagulants.
