Learning from Patient Safety Events

Synopsis: The last 12 months has seen significant shifts in the way healthcare organisations respond to harm, including but not limited to death, and this innovative training will explore how, by applying a socio-cultural perspective, organisations can actively restore trust when the unexpected happens.

By April 2024, new statutory regulations will come into force in England and Wales, which means all deaths in the community or acute settings that are not investigated by the coroner, will need to be scrutinised by a medical examiner. The new regulations ensure that bereaved families receive greater transparency and be given the opportunity to raise concerns after patient safety incidents occur.

You will be invited to discuss the existing barriers and consider alternative approaches to intervention within your own context. We will explore a number of effective practical frameworks and methods for generating a more open and transparent culture that uses all the knowledge in your networks to promote healing, organisational memory and learning.

This workshop is not about implementing PSIRF, but it will assist in refining your response framework using a restorative lens.

  • Apply new research, informed approaches, and best practice to enable your organisation to re-conceptualise “risk” and create an open, transparent and compassionate culture
  • Develop a nuanced and compassionate understanding of harmed families’ and carers’ lived experience
  • Learn how to engage and involve bereaved families and carers using a restorative lens
  • Learn how a culture of quality can enhance performance and reduce demand
  • Understand the principles of effective investigations.
  • Network with other practitioners and devise practical solutions for integrating across the care system.
